what happened was that gunner loaded a main gun shell, but wasn’t supposed to. so he tried to extract it, but it broke, and because there was already some shooting that day, propellant fell on bucket of hot stubs, igniting immediately. it was decided that it was fault of ammo that was not up to spec.
